What Causes Comedones? How Can They Be Treated?

Comedonal acne is a common dermatologic condition that affects many individuals. Although it may initially appear as small, non-inflammatory lesions, untreated comedones can progress to inflammatory acne, potentially leading to permanent acne scarring. Therefore, understanding the pathogenesis of comedones and appropriate treatment options is essential.
What Are Comedones?
Comedones are a type of non-inflammatory acne characterized by small papules, raised lesions, or subcutaneous bumps. They develop as a result of the accumulation of debris, desquamated keratinocytes, and excess sebum within the pilosebaceous unit.
Classification of Comedones
For appropriate management, comedones are classified into two major types as follows
- Open Comedones
Open comedones, commonly referred to as blackheads, are characterized by enlarged follicular openings with dark-colored plugs. These lesions can be mechanically extracted; however, improper extraction techniques may induce inflammation.
- Closed Comedones
Closed comedones, commonly known as whiteheads, are more prevalent than open comedones. They appear as small, skin-colored papules with closed follicular openings, making them difficult to express. Improper manipulation may result in inflammation and subsequent acne scarring
Treatment of Comedones
The treatment approach depends on the clinical type of comedones.
- Comedone Extraction
Both open and closed comedones may be removed through manual extraction. However, the procedure should be performed correctly to minimize the risk of inflammation and acne scar formation
- Pharmacologic Therapy
Pharmacologic treatment may include oral antibiotics such as tetracycline, doxycycline, and macrolides, as well as topical benzoyl peroxide. Medication should be prescribed and supervised by a physician or qualified healthcare professional to reduce the risk of adverse effects.
- Innovative Light-Based Therapy for Comedones and Inflammatory Acne
Light-based therapy is an effective treatment modality for comedones. It targets Cutibacterium acnes, regulates sebaceous gland activity, reduces comedone formation, and improves inflammatory acne. The procedure is non-invasive, painless, and does not require a recovery period
At THE KLINIQUE, the Acne Clear Program combines five treatment modalities to prevent inflammatory acne, minimize acne scarring, and manage comedones before progression. The protocol includes treatment with FDA-cleared NASA-standard LED light therapy, together with anti-inflammatory agents and acne medications that act synergistically to improve treatment outcomes
