Understanding Premature Facial Ageing and Sagging Skin

Air pollution, dust, smoke, lifestyle habits, and inadequate skincare can also cause younger people to experience premature skin ageing. Understanding the causes of an older-looking face is therefore an important first step towards choosing an appropriate method to restore a fresher and more youthful appearance.

What Causes an Older-Looking Face, Sagging Skin, and Wrinkles?

1. Increasing Age

As we age, the facial bones gradually lose volume and the skin becomes less elastic because of declining collagen and elastin levels.

Changes also occur within the facial fat layers, resulting in:

๐ Sagging skin

๐ Fine lines and wrinkles

๐ Deeper facial folds

๐ Dry and rough skin

๐ Reduced skin moisture

๐ A dull or tired-looking complexion

2. Sunlight and Pollution

Ultraviolet radiation from sunlight and environmental pollution are among the main factors that weaken the skin.

They may contribute to:

๐ Dullness

๐ Dark spots

๐ Melasma and freckles

๐ Uneven skin tone

๐ Dry and dehydrated skin

๐ Premature facial ageing

3. Insufficient Sleep

During sleep, the body produces melatonin, a hormone involved in the body’s recovery and repair processes. The period between approximately 10 p.m. and 2 a.m. is often considered important for physical restoration, particularly when the body reaches deep sleep.

Regularly staying up late or not getting enough sleep may prevent the skin from recovering fully, leading to:

๐ A tired complexion

๐ Dark circles beneath the eyes

๐ Dull-looking skin

๐ A worn-out appearance

๐ Premature signs of ageing

4. Smoking and Alcohol Consumption

Chemicals in cigarettes can damage collagen within the skin, contributing to premature wrinkles and skin deterioration.

Alcohol may also cause dehydration and reduce skin elasticity, making wrinkles and fine lines more visible.

5. Repeated Facial Expressions

Repeated facial movements, such as smiling, laughing, squinting, or frowning, can gradually create lines and deeper creases on the face.

Over time, these expression lines may become visible even when the facial muscles are relaxed.

6. Insufficient Water Intake

Water is essential for normal body function and skin hydration.

Not drinking enough water may cause:

๐ Dry and rough skin

๐ Reduced moisture

๐ A tired-looking complexion

๐ Dullness

๐ Loss of radiance

๐ Premature skin ageing

Ways to Restore a More Youthful-Looking Face

The most appropriate treatment depends on the area of concern, the degree of skin laxity, the facial structure, and the underlying cause of the older-looking appearance.

Four-Area Facelift with the Triple Lock Technique

The four-area facelift using the Triple Lock technique is one of the surgical options offered at ALINE Clinic for correcting significant facial ageing and sagging.

The technique was developed by Dr Phuwit Kongchayanan, a specialist in plastic and reconstructive facial surgery. It lifts and repositions tissues across three layers:

๐ The skin layer

๐ The subcutaneous tissue layer

๐ The SMAS layer

Lifting the deeper SMAS layer may provide clearer and more natural-looking results than procedures that tighten only the superficial skin.

Possible benefits include:

๐ Improving significant facial sagging

๐ Repositioning deeper facial structures

๐ Reducing wrinkles and deep folds

๐ Creating a firmer and more youthful appearance

๐ Providing longer-lasting results than superficial lifting techniques

The Triple Lock technique also focuses on placing and concealing incisions carefully. Once the wounds have healed completely, the scars may become difficult to notice.

Results may last for up to approximately ten years, depending on the individual’s age, skin condition, lifestyle, and natural ageing process.

Sub-Brow Lift

A sub-brow lift is a surgical procedure designed for people whose face appears older because of drooping eyebrows, loose upper-eyelid skin, or sagging around the outer eye area.

This procedure is not limited to older adults. Younger people may also choose a sub-brow lift to make the eyes appear brighter, sharper, and more youthful.

During the procedure, the surgeon creates an incision beneath the eyebrow and removes excess skin and muscle from the upper eyelid area. The incision is then closed and concealed along the lower border of the eyebrow.

Possible results include:

๐ Firmer upper eyelid skin

๐ A clearer eyelid crease

๐ A lifted outer eye area

๐ Brighter-looking eyes

๐ A naturally younger appearance

Long-Incision Double-Eyelid Surgery

Some people appear older because of concerns around the eyes, including loose eyelid skin, drooping eyelids, or weakened eyelid muscles.

Long-incision double-eyelid surgery can be adjusted according to the specific condition.

For Drooping Upper Eyelids and Outer Eye Corners

The surgeon creates an incision extending from the inner to the outer eyelid and removes excess skin.

The procedure may help:

๐ Lift the upper eyelids

๐ Raise the outer eye area

๐ Create a clearer eyelid crease

๐ Make the eyes appear larger and more open

๐ Restore a fresher appearance

For Weak Eyelid Muscles

When eyelid muscle weakness is present, double-eyelid surgery may be performed together with correction of the weakened eyelid muscle.

The procedure reaches the muscular layer to address the underlying cause of drooping or sleepy-looking eyes.

Possible results include:

๐ Improved ability to open the eyes

๐ Reduced eyelid drooping

๐ A clearer eyelid crease

๐ A more appropriate eyelid height

๐ Reduced appearance of thick or excessively deep eyelid folds

๐ A more balanced and alert-looking eye area

Under-Eye Bag Removal Combined with a Mid-Facelift

Under-eye bags are another common concern that can make the face look older and more tired. They may affect older adults as well as younger working-age individuals.

Some people have under-eye bags together with:

๐ Sagging mid-facial skin

๐ Fine lines and wrinkles

๐ Reduced skin firmness

๐ A tired facial appearance

Under-eye bag removal combined with a mid-facelift is designed to correct both areas during one surgical procedure.

The treatment may help:

๐ Remove or reposition excess under-eye fat

๐ Improve loose lower-eyelid skin

๐ Lift sagging tissue in the middle of the face

๐ Reduce the tired appearance caused by under-eye bags

๐ Restore a more youthful appearance to both the eyes and face

Open Rhinoplasty

The tissues of the nose also change with age. Skin loses elasticity, while the strength and support of the nasal bones and cartilage may gradually decline.

These changes may cause the nasal tip to droop, making the face appear older.

Open rhinoplasty allows the surgeon to access and adjust the internal nasal structure directly. At ALINE Clinic, three main open-rhinoplasty techniques are available.

1. Silicone-Free Open Rhinoplasty

This technique corrects the internal nasal structure and increases the height of the bridge and tip without using a silicone implant.

The treatment plan is designed according to each patient’s nasal anatomy and concerns.

2. Hybrid Open Rhinoplasty

The hybrid technique combines several materials and surgical methods.

An I-shaped silicone implant is used only to increase the height of the nasal bridge. No silicone is placed in the nasal tip.

The nasal tip is reshaped and supported with:

๐ At least two layers of ear cartilage

๐ One layer of fatty tissue

The surgeon considers the desired nose shape and the amount of tissue at the nasal tip to create a result that meets the patient’s preferences while prioritising safety.

This technique may help reduce direct pressure from silicone on the nasal tip and lower concerns about thinning tissue or implant exposure.

3. GDCC Open Rhinoplasty

GDCC stands for Glued Diced Costal Cartilage.

This technique originated in South Korea and combines open rhinoplasty with structural correction using finely diced rib cartilage and the patient’s own tissue instead of silicone.

The technique can be used to:

๐ Correct structural nasal concerns

๐ Increase the height of the nasal bridge

๐ Improve nasal-tip projection

๐ Reshape the nasal framework

๐ Reduce concerns associated with silicone implants

Because the procedure uses the patient’s own cartilage and tissue, it may provide a projected and well-defined nasal tip without placing a silicone implant inside the tip.

Choosing the Right Treatment for Facial Ageing

An older-looking face can result from several different structural and skin-related concerns. The most appropriate treatment therefore depends on which area is creating the ageing appearance.

๐ Significant facial sagging may require a deeper facelift technique.

๐ Drooping eyebrows or loose upper-eyelid skin may benefit from a sub-brow lift.

๐ Eyelid sagging or weak eyelid muscles may require double-eyelid surgery with structural correction.

๐ Under-eye bags combined with mid-facial sagging may be treated with under-eye surgery and a mid-facelift.

๐ A drooping nasal tip may be corrected with open rhinoplasty.

A detailed assessment of the skin, muscles, fat layers, bone structure, and individual expectations is important before selecting a surgical procedure. The treatment should be designed specifically for the patient to achieve a balanced, natural-looking, and appropriately youthful result.
