What is IUI? A Detailed Understanding of Intrauterine Insemination

IUI (Intrauterine Insemination), also known as intrauterine artificial insemination, is a fertility treatment method in which sperm from the male partner (or donor sperm) that has been processed and prepared in a laboratory is directly injected into the woman’s uterine cavity through the cervix during the time of ovulation.

The main principle of IUI is to shorten the distance and bypass obstacles at the cervix, allowing a higher number of strong, motile sperm to travel more easily and quickly to meet the egg in the fallopian tube. This increases the chances of fertilization and natural pregnancy.

Preparation Before Undergoing IUI to Increase the Chances of Success

Although IUI is not a complicated medical procedure, preparing the body in advance both for the male and female partner is an important factor in enhancing overall health and increasing the likelihood of pregnancy. Doctors commonly recommend the following:

- Female partner preparation: It is recommended to start taking folic acid at least 1–3 months in advance to prepare the body and help prevent neural tube defects in the baby. In addition, adequate rest, stress management, and maintaining a healthy body weight within a normal BMI range are important, as they positively affect ovarian response.

- Male partner preparation: Alcohol consumption and smoking should be strictly avoided, as toxins directly impair sperm quality and motility. Importantly, before the scheduled insemination day, the male partner should abstain from ejaculation for approximately 2–7 days (as advised by the physician) to obtain the most suitable sperm count and concentration for selection.

- Lifestyle adjustments for both partners: Both should focus on consuming a nutrient-rich diet, particularly protein, vegetables, and fresh fruits. Regular moderate exercise without overexertion helps stimulate efficient blood circulation.

Self-Care After IUI: Making the Waiting Period Meaningful

After the procedure, you can return to your normal daily activities immediately. However, a few recommendations may provide reassurance:

- Avoid strenuous activities: Although bed rest is not required, avoiding heavy lifting or high-impact exercise during the first 1–2 weeks can help prevent excessive fatigue.

- Monitor your symptoms: Light spotting or mild lower abdominal discomfort may occur and is considered normal. However, if any unusual symptoms arise, you should consult your case coordinator or medical staff immediately.

- Positive mindset matters: The 14 day period before a pregnancy test (the “2-Week Wait”) can cause anxiety. Engaging in relaxing activities and avoiding testing too early can help reduce stress.

Reasons and Indications for Undergoing IUI Treatment

The decision to proceed with IUI usually occurs when attempts to conceive naturally have not been successful within an appropriate timeframe (typically 1 year for general couples, or 6 months for women aged 35 and above). Key medical reasons that make IUI an appropriate option include:

- Mild male factor infertility: In cases where semen analysis shows lower-than-normal sperm concentration or reduced motility, IUI helps select only strong, highly motile sperm and delivers them directly into the uterine cavity, reducing the need to travel through natural barriers.

- Cervical factor infertility: Some women may have abnormally thick cervical mucus or an acidic environment that hinders sperm movement. IUI bypasses this area entirely, reducing sperm loss before reaching the target.

- Ovulation disorders: For women with irregular ovulation or irregular menstrual cycles, IUI is often combined with ovulation-inducing medications and ultrasound monitoring to ensure insemination occurs precisely at the time of ovulation something that can be difficult to time accurately through natural methods.

- Unexplained infertility: In many cases, both partners’ test results appear normal, yet pregnancy does not occur. Physicians may recommend IUI as a first step because it increases the chance of fertilization compared to natural conception, while remaining low-risk and more affordable than in vitro fertilization (IVF).

- Physical or lifestyle limitations: For example, male ejaculation issues or couples whose schedules do not align with the ovulation period. IUI helps medically optimize timing for the highest chance of success.

Advantages of IUI

- Natural and minimally invasive: The procedure closely resembles natural conception and does not require surgery or anesthesia.

- Cost-effective: The cost is significantly lower than IVF or ICSI.

- Quick procedure: The insemination process takes only a short time, and normal daily activities can be resumed immediately afterward.
