Description
Chemotherapy often comes with complications that can impact a patient’s health and quality of life. Therefore, the hospital has developed a “Preparation Program” to help reduce risks and enhance comprehensive care. This program is divided into six key steps:
1. Pre-Treatment Educational Videos
The hospital has produced an informational video for patients and their families, explaining the treatment process in an easy-to-understand manner.
The content provides information on potential side effects, such as neutropenic fever, which occurs in 12% of certain patient groups. If not treated promptly, serious complications can lead to death.
2. Pre-Treatment Knowledge and Understanding Assessment
Patients are provided with a questionnaire and an information pamphlet before chemotherapy to assess their understanding of side effects and self-care after discharge.
The program’s results show an average improvement in cognitive skills, from 6 items to 10 items (based on a 20-item Ped. Test).
3. One-Page Documentation: Key Information and Contact Details
The multidisciplinary team has developed a one-page document outlining any unusual symptoms to watch for, initial management methods, and emergency contact information via Line OA or the nursing team’s phone number. Patients and families can use this as a self-care guide after receiving medication.
4. QR Code for Post-Medication Symptom Tracking
Patients can scan the QR Code to report symptoms such as fever, nausea, loss of appetite, or other abnormalities.
The treatment team will use the information to proactively monitor symptoms and provide advice without waiting for an appointment.
5. Home Early Detection via QR Code
Create a QR Code for home symptom monitoring during the risk period to assess symptoms from the beginning.
If any dangerous indicators, such as fever, chills, or low blood counts, are detected, the nursing team will contact you immediately to provide assistance, reducing the risk of emergencies and hospital readmissions.
6. Program Performance Monitoring
The hospital collects program usage data to assess treatment quality and patient satisfaction.
The results will be used to develop improved care guidelines and serve as a model for other service units.
