Description
A neck lift that fails — or looks acceptable at rest but reveals persistent turkey neck when looking down — is often not a matter of poor healing. It is a matter of insufficient depth.
At Dr. Gorn Aesthetique, revision neck lift begins with one fundamental question: why did the previous procedure fail?In most cases, the answer lies in inadequate management of deep cervical structures beneath the SMAS layer. Procedures focusing solely on platysma tightening and skin pulling may produce temporary improvement — but without addressing the true anatomical root, results will not hold, particularly in the chin-down position. ❌
Common Causes of Neck Lift Failure
▶️ Unaddressed deep cervical structures — enlarged subplatysmal muscles, submandibular glands, and deep cervical fat left intact, allowing neck contour to relapse
▶️ Incomplete platysma management — effective correction requires addressing the platysma in all dimensions: front, lateral, superior, and inferior. The upper portion must be repositioned to conform to the submandibular contour; the lower portion tightened from both front and back. In select cases, selective sectioning of terminal cervical nerve branches is performed to reduce active muscle pull and minimize platysmal band recurrence
▶️ Skin-only or superficial tightening — surface pulling without deep structural release leads to early recurrence, especially chin-down
▶️ No posterior correction when indicated — post-auricular incisions are essential for redefining the lateral jawline from neck to jaw angle; without them when indicated, jaw definition remains incomplete regardless of anterior correction
Our Revision Approach
✅ Deep structural assessment — identifying subplatysmal muscle hypertrophy, submandibular gland enlargement, and residual deep fat as primary causes of recurrence ✅ Deep neck surgery below the SMAS — subplatysmal muscles, submandibular glands, and deep fat corrected through a submental incision ✅ True 3-dimensional platysma management — front, lateral, superior, and inferior with selective cervical nerve sectioning ✅ Posterior neck lifting — hidden post-auricular incisions for lateral repositioning and sharp jawline definition ✅ Chin-down position as the benchmark — all correction evaluated at the most challenging angle
Suitable For ✔ Persistent turkey neck or double chin after previous neck lift ✔ Results that fail in the chin-down position ✔ Cases where only platysma tightening or skin pulling was previously performed ✔ Unaddressed submandibular gland or subplatysmal muscle bulk ✔ Men and women seeking definitive, long-lasting neck contour correction
