What is SCULPTRA?

SCULPTRA is an injectable collagen biostimulator composed of PLLA (Poly-L-Lactic Acid) microparticles. It helps stimulate collagen production, restore volume, and lift and firm the face. The results can last up to 2 years.

SCULPTRA specifically stimulates the production of Collagen Type I, which is the most abundant type of collagen in the body. There are more than 16 types of collagen that the body can produce, and Type I accounts for over 90% of the body’s total collagen. It plays a key role in improving skin elasticity and supporting wound healing. Collagen Type I is found in the skin, hair, bones, connective tissues, and blood vessel walls.

SCULPTRA has been shown to increase Collagen Type I production by up to 66.5% at 3 months after treatment.

PLLA is considered the first collagen biostimulator approved by the U.S. Food and Drug Administration (FDA) in 1999. It is a biocompatible, plant-derived synthetic material that is safe and biodegradable in the body. PLLA has long been used in medical applications, such as dissolvable sutures, screws, and plates for bone fixation.

By stimulating collagen production deep within the skin, SCULPTRA helps restore underlying structural support, reduce wrinkles, improve skin firmness, and promote a more youthful appearance.

Mechanism of Action of SCULPTRA

Before injection, SCULPTRA is reconstituted with sterile water and then injected into the subcutaneous layer of the skin, primarily in areas such as the temples, cheeks, and lateral areas beneath the cheekbones. Once injected, the product disperses evenly throughout the tissue.

The PLLA particles stimulate an increase in fibroblasts, the cells responsible for producing two essential structural proteins: collagen and elastin. These proteins help strengthen the skin’s internal framework and support the dermis, the middle layer of the skin composed of elastic connective tissue, where fibroblasts are abundant.

Over time, PLLA is gradually metabolized into lactic acid, a compound in the same family as AHAs. This process supports gentle skin renewal by promoting the shedding of dead skin cells and revealing smoother, more radiant skin.

Additionally, PLLA stimulates collagen synthesis through the involvement of white blood cells called macrophages, which help remove foreign particles from the body. As macrophages process the PLLA microparticles, the material slowly degrades. However, the newly formed collagen remains and continues to accumulate.

As a result, wrinkles gradually soften, deep folds and depressions appear shallower, and overall skin firmness improves. Some patients may begin to notice subtle changes as early as the first week after treatment, with continued collagen stimulation and progressive improvement over time.
