What is Vascular Access?
Vascular access for hemodialysis refers to the pathway through which blood is brought from the patient's body to the filter and cleaned blood is returned. There are two types of vascular access: central venous access catheters and permanent vascular access surgery.
Maintenance of Central Venous Access Catheter or Plastic Access for Hemodialysis:
Keep the wound clean and avoid touching or removing the dressing.
Prevent the wound from getting wet; seek treatment if it peels off or becomes wet.
Avoid wearing o-neck t-shirts to prevent obstruction of vascular access.
Consult a physician in case of abnormalities or issues.
Patients with Permanent Catheter:
Arteriovenous fistula is preferred as it offers fewer complications and longer useful life.
Arteriovenous graft may be used if the coronary artery is small, placed under the skin for use within 2-4 weeks.
Patients should follow self-care instructions for a successful recovery.
Self-Care Guidelines:
Elevate arms to reduce edema during the first 3 days after surgery.
Start gentle exercises on the arm without pain or bleeding after 3 days, using a rubber ball.
Avoid wetting the wound, touching or hitting surgical sites.
Do not use the arm for pressure measurement, blood drawing, or IV fluids.
Follow prescribed antibiotics if given after surgery.
During Hemodialysis:
Avoid wearing wrist accessories or shirts that constrict the arm.
Clean the access area before treatment.
Regularly examine the access area for any changes.
Refrain from rubbing the skin near the access site.
Follow exercise recommendations using a rubber ball.
Avoid pressure measurements, blood drawing, and IV procedures on the access arm.
