At Ubonrak Thonburi Hospital, our Life Insurance & Compulsory Motor Insurance (Por Ror Bor) Department provides consultation and support for the use of health insurance benefits for both Thai and international patients. Illness can be unpredictable, and we aim to ease your concerns by helping you navigate insurance coverage during your hospital visit.

Important Note: Before receiving treatment, please contact us to confirm whether your insurance company is in our approved network.

Required Documents for Insurance Verification:

Please bring the following for verification upon arrival:

• Thai National ID Card

• Insurance Card or Policy Number

• Name Change Documentation (if applicable)

• Birth Certificate (for newborns to 6-year-olds)

• Any official government-issued identification

Outpatient (OPD) Services:

1. Patient must sign a consent form at the medical records department before receiving treatment.

2. Patient must also sign the insurance claim form and provide a copy of their ID card.

3. Any amount not covered by insurance must be paid by the patient directly.

Inpatient (IPD) Services:

1. Patient or family member must book a room with the medical records department.

2. Hospital staff will verify the patient’s insurance eligibility with the provider.

3. The hospital will fax the insurance claim (FAX CLAIM) to the provider on the discharge day. Response time is typically 1–2 hours. If additional medical history is requested, it may take 1–2 more hours.

4. Any charges not covered by the insurance must be settled by the patient.
