Services & Treatments

1) Diabetic Retinopathy Management

Overview: High blood glucose damages retinal microvasculature, potentially leading to severe vision loss if untreated. Treatment depends on stage and severity.

1.1 Intravitreal Injections

  • Agents (as clinically appropriate): Faricimab-svoa (Vabysmo), Ranibizumab (Lucentis), Aflibercept (Eylea), Bevacizumab (Avastin)

  • Goal: Suppress VEGF to reduce vascular leakage/neovascularization and macular edema

  • Schedule: Often loading doses (e.g., monthly initially) then tailored by the retina specialist

1.2 Retinal Laser Therapy

  • Focal/Grid Laser:

    • Process: Seal leaking microaneurysms; reduce macular edema

    • Pros: Outpatient, quick, minimal downtime

    • Limitations: Best for selected non-proliferative stages; repeat sessions may be needed

  • Panretinal Photocoagulation (PRP):

    • Process: Peripheral laser to regress pathologic neovascularization and prevent complications (vitreous hemorrhage, tractional RD)

    • Pros: Reduces risk of severe vision loss

    • Trade-offs: Possible reduced peripheral/night vision in some patients

Preparation & Aftercare

  • Before: Pupil dilation; topical anesthesia as needed

  • During: Brief flashes of light are expected

  • After: Temporary blur/photopsia may occur and resolves within hours

  • Advice: Avoid driving/precision tasks for 24 hours; wear sunglasses outdoors; seek care urgently for pain, redness, or sudden vision drop

Note: Laser is one component of a holistic plan—systemic control of glucose and blood pressure and regular eye exams remain essential.

2) Vitrectomy (Vitreoretinal Surgery)

Common Indications

  • Non-clearing/recurrent vitreous hemorrhage

  • Retinal detachment (especially tractional)

  • Epiretinal membrane/traction at the macula

Care Pathway

  • Pre-op: Comprehensive assessment (ocular, labs, systemic), risk/benefit discussion; local anesthesia or sedation per case

  • Intra-op: Small sclerotomies; remove opacified vitreous/blood; peel membranes; gas or silicone oil tamponade when indicated

  • Post-op: Typically 1–2 days inpatient observation; strict adherence to drops, systemic control, and scheduled follow-ups

Techniques

  • Pars Plana Vitrectomy (standard 3-port)

  • Microincision Vitrectomy (<0.5 mm ports) for reduced tissue trauma and potentially faster recovery when appropriate

Risks/Considerations

  • Endophthalmitis, IOP elevation, cataract progression, among others—surgical decisions balance potential benefits against risks in shared decision-making.

 

Red-Flag Symptoms (Seek Care Urgently)

  • New flashes/floaters with a curtain-like shadow

  • Rapid distortion or drop in vision

  • Pain, redness, or vision loss after treatment

Self-Care & Follow-Up

  • Adhere strictly to prescribed eye drops/medications

  • Optimize systemic control (glucose, BP, lipids)

  • Keep all scheduled visits—even if symptoms improve
