Description

The GI Motility Unit at Bangkok Hospital specializes in the diagnosis and treatment of gastrointestinal motility disorders. Our team of experts, supported by cutting-edge technology and international standards of care, provides comprehensive services for various conditions related to the upper and lower gastrointestinal tracts. Some of the conditions we address include swallowing difficulties (dysphagia), gastroesophageal reflux disease (GERD), undiagnosed abdominal pain or discomfort, and constipation and fecal incontinence. 

For swallowing difficulties (dysphagia), we offer advanced diagnostic techniques such as high-resolution esophageal manometry. This procedure allows us to assess the movement and pressure exerted by the esophageal muscles and sphincter muscles during swallowing. We also utilize high-resolution esophageal manometry with impedance to evaluate esophageal motility and the movement mechanism of the lower esophageal sphincter. This helps in accurately diagnosing swallowing difficulties and identifying the types of residual substances that may be causing the issue. 

To diagnose and determine the severity of gastroesophageal reflux disease (GERD), we provide pH monitoring tests. These tests measure the amount of acid flowing into the esophagus from the stomach, allowing us to better understand the pattern of acid reflux. We offer both catheter-pH monitoring and wireless capsule Bravo pH monitoring options to suit individual patient needs. 

Our unit also focuses on diagnosing undiagnosed abdominal pain or discomfort. We use hydrogen breath tests (HBT) to diagnose small intestine bacterial overgrowth and carbohydrate malabsorption. This safe and easy-to-perform test helps identify the root causes of abdominal pain, discomfort, or tenderness after meals, enabling patients to adjust their eating habits accordingly. 

For patients experiencing constipation and fecal incontinence, we offer high-resolution anorectal manometry. This procedure evaluates the causes of constipation and fecal incontinence by measuring the pressures of the anal sphincter muscles, the sensitivity of the rectum to stool movements, and the neural reflexes during the resting state of the sphincter. We also provide 3D high-definition anorectal manometry, which provides detailed topographic images of pressure along the anal canal, aiding in the identification of abnormalities. 

In addition, our unit offers anorectal biofeedback training as a treatment program for patients with chronic constipation or fecal incontinence. This program utilizes instrument-based and neuromuscular conditioning techniques to train muscle functions and achieve regular bowel movements. Through the use of anal probes, we measure the pressure profile of the rectal sphincter muscles, allowing patients to learn effective techniques for healthy bowel movements.
