Management and Treatment of Dental Caries

Dental caries is one of the most common oral diseases worldwide. It is characterized by the destruction of dental hard tissues caused by bacterial activity within dental plaque. In addition to professional dental treatment, appropriate oral hygiene practices are essential for the prevention and management of dental caries. Early intervention is recommended, as untreated carious lesions may initially appear minor but can progress if left unmanaged.

Etiology of Dental Caries

Dental caries develops when dental plaque is not adequately removed. Oral bacteria metabolize dietary sugars and produce acids that demineralize tooth enamel. Without appropriate treatment, dental caries may progressively destroy tooth structure.

Clinical Manifestations of Dental Caries

In the early stage, dental caries may be asymptomatic because enamel does not contain sensory nerve endings. As the lesion extends beyond the enamel into deeper tooth structures, patients may develop increased sensitivity to hot, cold, sweet, sour, or sticky foods.

As the lesion progresses through the enamel, carious destruction may accelerate. The enamel surface may appear intact because of its hardness; however, the underlying lesion may enlarge progressively.

Once bacteria invade the dentin and reach the dental pulp, inflammation may develop, resulting in severe toothache. Patients may also experience pain during mastication, gingival swelling, halitosis, unpleasant taste, and, in advanced cases, abscess formation. Untreated infection may spread to adjacent tissues.

Treatment of Dental Caries

When carious lesions are confined to the enamel, treatment options include maintaining good oral hygiene practices, regular toothbrushing, and the use of fluoride-containing oral care products, including fluoride toothpaste, fluoride gels, and fluoride mouth rinses.

Once the lesion extends into dentin and cavitation is present, restoration with dental filling materials may be required. If substantial tooth structure has been lost, the dentist may recommend a dental crown to restore tooth morphology and function.

Teeth with pulpal involvement represent the most advanced stage of dental caries. Depending on the extent of the disease, treatment options include root canal therapy or tooth extraction. The dentist will determine the most appropriate management for each individual case.
