ICSI (Intracytoplasmic Sperm Injection) is an advanced assisted reproductive technology developed from IVF. It plays a crucial role, especially in cases of male-factor infertility. This article explains what is ICSI , who it is suitable for, how the procedure works, as well as its advantages and limitations, to help couples make informed decisions.

What Is ICSI?

ICSI is a fertilization technique performed outside the human body. An embryologist selects a single healthy sperm and injects it directly into the egg using a specialized microscopic needle to achieve fertilization.

This method overcomes situations where sperm cannot penetrate the egg naturally and significantly increases fertilization rates, particularly in couples with severe male infertility.

Who Is ICSI Suitable For?

ICSI is recommended for couples experiencing infertility in the following situations:

- Very low sperm count (severe oligospermia)

- Poor sperm motility or non-motile sperm

- A high percentage of abnormally shaped sperm

- Previous failure of fertilization with conventional IVF

- Need to use surgically retrieved sperm (such as PESA or TESE)

- Unexplained infertility

- Couples seeking to maximize fertilization rates in a treatment cycle

ICSI Procedure Steps

The ICSI process is similar to IVF, with the key difference occurring during fertilization. The procedure generally consists of five main steps:

1. Ovarian Stimulation

Hormonal medications are administered to stimulate the ovaries to produce multiple eggs, increasing the chances of obtaining high-quality eggs.

2. Egg Retrieval

Once the eggs have matured, they are retrieved through the vaginal canal under ultrasound guidance. This is a safe and commonly performed procedure.

3. Sperm Selection and Injection (ICSI)

An embryologist carefully selects the healthiest sperm and injects a single sperm directly into each egg to achieve fertilization.

4. Embryo Culture

The fertilized embryos are cultured in the laboratory for 3–5 days, allowing embryologists to select the best-quality embryos.

5. Embryo Transfer

The selected embryo(s) are transferred into the uterus, where implantation may occur, leading to pregnancy.

What Is ICSI

ICSI Success Rates

The success rate of ICSI depends on several factors, including:

- The woman’s age

- Egg quality

- Uterine health

- Experience of the medical team and laboratory

While ICSI improves fertilization rates, it does not guarantee pregnancy in every case.

Advantages of ICSI

- Significantly increases fertilization rates in cases of severe male infertility

- Requires only a very small number of sperm

- Suitable for couples with previous IVF fertilization failure

- Can be combined with genetic embryo screening (PGT)

Limitations and Disadvantages of ICSI

- Higher cost compared to conventional IVF

- Requires highly skilled embryologists and advanced laboratory facilities

- No 100% guarantee of pregnancy success

- Potential risks related to micromanipulation of eggs at the cellular level
