Description

Percutaneous Nephrolithotomy (PCNL) is indeed a minimally invasive method for the treatment of kidney stones, especially large ones, multiple stones, or stones in locations that are hard to reach through other procedures. 

PCNL involves the removal of kidney stones through a small puncture wound (around 1 cm) in the skin. It's performed under general or regional anesthesia. The procedure is typically guided by fluoroscopy (a type of X-ray), and an endoscope (a flexible tube with a light and camera) is used to locate and remove the stones. 

Here's a step-by-step breakdown of the procedure: 

  • The patient is positioned so that the stone can be easily accessed. 

  • Under fluoroscopic guidance, a needle is inserted through the skin and into the kidney. 

  • A wire is then inserted through the needle, and the needle is removed. 

  • A series of dilators are used to create a tunnel to the stone. 

  • An endoscope is passed through this tunnel to visualize the stone. 

  • Once the stone is seen, it can be fragmented using various techniques such as ultrasonic lithotripsy, pneumatic lithotripsy, or laser lithotripsy. 

  • The fragments are then removed using a special basket or suction device. 

  • Once all the fragments are removed, a tube (nephrostomy tube) is left in the kidney to drain urine and any residual stone fragments. This tube is typically removed a few days after the procedure. 

The advantages of PCNL over open surgery or Extracorporeal Shock Wave Lithotripsy (ESWL) are numerous, including a higher success rate for large and complex stones, shorter hospital stays, less pain, and faster recovery. It's important to note, however, that like all surgical procedures, PCNL does come with risks, including bleeding, infection, injury to surrounding organs, and the possibility of residual stones. Therefore, it's essential that patients thoroughly discuss the procedure and its potential risks and benefits with their healthcare provider before deciding on the best treatment approach.
