Dr. Netnipa Phromnarat, a specialist obstetrician and gynecologist at Hua Chiew Hospital, said that uterine tumors are common in women aged 30-50 years. They occur due to abnormal changes in the muscle cells of the uterus, which can develop either in the uterine wall or within the uterine cavity. Most cases show no symptoms, but if the tumor occurs in the uterine cavity, it may cause heavier menstrual bleeding than normal.
Cause: The exact cause of uterine tumors is not yet fully understood, but there are factors that are associated with the condition, including genetics or a family history of tumors, and elevated estrogen levels. It has been found that when women reach menopause, a period when estrogen levels decrease, uterine tumors tend to shrink as well.
Symptoms
- Severe menstrual pain or heavy periods : This occurs due to a tumor protruding into the uterine cavity, causing the uterus to enlarge and the surface area of the cavity to widen.
- Frequent urination : Large tumors may press on the bladder, causing increased urinary frequency.
- Constipation : The tumor may press against the rectum, leading to constipation.
- Infertility : This can occur if the tumor grows into the uterine cavity or causes a blockage in the fallopian tubes, interfering with embryo implantation.
Treatment: The treatment depends on the patient's symptoms, the characteristics of the tumor, its growth, and other factors such as the patient's age and desire to have children.
Medication: Since there is currently no medication that can completely cure uterine tumors, doctors use medications to alleviate symptoms. These may include pain relievers, anti-inflammatory drugs, birth control pills, and tranexamic acid to reduce menstrual bleeding.
Minimally Invasive Surgery:
- Hysteroscopic Surgery: This procedure is used to treat uterine tumors within the uterine cavity. A camera is inserted through the vagina and cervix into the uterus, and an electrical wire is used to remove the tumor. This method does not involve external incisions, allowing for a quicker recovery.
- Laparoscopic Surgery: This method is used for larger uterine tumors and is comparable to traditional abdominal surgery. In laparoscopic surgery, small incisions (0.5-1 cm) are made (usually 3-4 incisions), allowing for faster recovery with less pain and minimal scarring, helping the patient return to daily activities quickly.
